The remaining late-career levers

  • CSFA if not yet pursued. Even at year 10, the $20-30k annual premium pays back the program cost within 12 months and compounds for the remaining career.
  • Specialty deepening. Cross-train into a second specialty (CVOR + transplant; robotics urology + general) to access higher-pay rotations.
  • Federal employment. VA, IHS, and military civilian positions reward tenure with predictable step increases and pension benefits unavailable in the private sector.
  • IC arrangements (CSFA only). At senior tenure with CSFA, IC at high-volume ortho or plastic ASCs frequently delivers $115-150k.
  • Surgical services management. The path from charge tech to surgical services coordinator to director typically requires a bachelor degree but reaches $95-130k.
